The Consultation Form has been created to give the Traditional Chinese Herbal Medicine practitioner an overview of your body’s unique internal processes through the eyes of Chinese Herbal Medicinal Philosophy and Methodologies.
The Consultation Form will ask you to report the quality of your health involving aspects of your body including:
- emotions
- energy levels
- sleep habits & quality
- heart palpitations
- sleep quality
- elimination
- appetite
- digestion
- cravings
Minor details are extremely helpful for the Chinese Medicine Herbalist. This format can also be used to help a loved one, friend and even your dog in getting personal assistance.
Once you submit the Consultation Form, the Chinese Herbalist will take the necessary amount of time to assess your unique Chinese Medicine Patterns. The assessment will include research time to get a complete picture of your whole system, to determine the different layers of disharmony that exist and how they interrelate. Please allow up to seven business days for the Chinese Herbalist to review your pictures and Consultation Form, though the review may happen quicker.
